When you send a request through this website we collect your name, phone number, email address and whatever you choose to tell us about why you're getting in touch.
We use it to call you back and arrange an appointment. That's all. We don't sell it, rent it or pass it to anyone outside the practice.
The form on this site is not a secure channel for protected health information. Keep it to the basics and we'll take the rest properly once we speak.
Health information you give us once you're a patient is covered by HIPAA and our Notice of Privacy Practices, available at the office.
